One possible explanation is that the image was photoshopped either completely or partially.

If we assume the artifact is real and not manipulated in any way, then it's likely one of the two.

1. The material is bunching up there due to the way the dress fits. I tend to have this problem with pants.
2. She has heavy periods and is wearing a long pad because of it. Sometimes walking can cause them to creep forward or backwards, and stick out if the material isn't sufficiently restrictive.

In all the other photos I kinda think maybe she has a colostomy bag cause the creases in her clothes go higher up in some.

Well....I asked what it might be and that second one is a possibility I had never thought of.

Omg, I can't believe I'm posting this, but I'm not the one who asked. I will also say I do not believe that Joan Rivers would lie about such a thing. Another thing (since I'm Miss Conspiracy Theory) :mrgreen: , Joan Rivers ended up dying a few months after this came out. Here goes and y'all decide for yourselves:
