Need Help with Wording a Letter

I am on a rental assistance program which is for disabled people. I recently found that I can add someone to the lease if my doctor writes a letter saying that I need them here. (Which I definitely do. There are days when I am so fatigued that I have to stay in bed) I want it to be my SO, because I already know him and don't want a stranger living here.
I already got the letter from my doctor. Now I have to write to my landlord requesting permission to add a second person to the lease. I have heard that there is a term that is used when a person lives with a disabled person and looks after them, but I am not sure of the correct term. I heard someone call it PCA (personal care assistant) but that might be a person who is especially trained and I am not sure if that is what having a friend who is helping to care for one is called.
Does anyone know what the term is? Or, has anyone else had to have someone move in to take care of them or do you know of someone who went through this procedure with Section 8?
I would appreciate feedback.

I'd recommend going businesslike, polite and as brief as possible:
Dear Mr Jones. I am seeking your permission for John Doe to reside at this address along with myself. I am including a letter from my physician for reference. Best Regards, Jane Smith
This might acheive your objective at once. Or it may turn out to be just the 1st part of a negotiation. If your landlord has questions or qualifications which need to be met, this short format allows for him to state what is relevant.
Approaching any negoiation in a positive way is your best chance of getting what you need.

Yup. Short and sweet. Any business letters should stick to the point.
The landlord may want to raise the rent. Depending on your current lease he may be well within his rights. Have you gone through the current lease papers with someone? It helps to know exactly what you've signed up for.

The landlord may want to raise the rent. Depending on your current lease he may be well within his rights. Have you gone through the current lease papers with someone? It helps to know exactly what you've signed up for.
I have been told that the rent will be recalculated by the rental assistance people if someone moves in, because they have to take both our incomes into consideration. But it shouldn't go too high, because it is a rental assistance program.
