Tips on how to maintain motivation on workout routine?

I have started working out many times, only to fall off the wagon less than two weeks later and not get back on a couple of months later.

Starting to work out is easy while making it a healthy habit is anything but.

So..... What kind of mental gymnastics should I try out to keep up the motivational mojo for more than a couple of weeks?
