I'm still not sure what the problem is for you, but "re-read the book" has to be some of the dumbest advice a teacher can give you.

If you do very well on reports then you must be having a problem with tests? Do you take notes while you read?

What specifically do you have a hard time with in interviews?

I wouldn't worry too much about "disclosing" anything during an interview (meaning I wouldn't do it).

This is not necessarily true. As others have said, something may have gotten in the way, or it might have slipped her mind.
