Idea for a thread.. help needed

I’ve got an idea i’ve been writing out for a thread in the Adolescent Autism Forum.
It’s gonna be a thread to find friends your age since i see a lot of threads of teens lookin for friends.

Ok here’s what i need help with: the introduction form thing.

Currently the introduction form thing is:

Age:

Hobbies:

Interests:

And then you can add whatever else about you or your interests that you want.

Is this good? Should there be other stuff for the form base? I didn’t add “Name:” since I myself don’t like sharing my name online sooo
