It depends; maybe "Oh my goodness!" if it's loud and they surprised me, or "bless you" or nothing.

People don't need to say ''bless you'' when I sneeze because my sneezes are caused by an air bubble getting trapped in the depths of my sinuses (I was born with narrow sinuses, you can even see small bump on the top side of my nose), and so it's just my body's way of getting rid of it, so my sneezes are harmless to other people.

But still, it is just considered polite to say ''bless you'' when people sneeze.

"Bless you" still works. We don't really need a new saying...

Other than that, just saying nothing is ok, depending on the person.

In Germany, people find it funny to wish people "Schönheit" (beauty) or "Verstand" (intelligence) instead of Gesundheit, depending on what the sneezer appears to need most And my grandfather often jokingly said "Verreck du Aas" (die, you bugger). Another common reply is "Aufwischen" (mop that up).
