Sensory integration/processing assessment forms?

Does anyone know if I can find any sensory processing forms online? Like a checklist that I could print off? I'd sort of like to see where my 12 year old son falls. I mean, there are some real obvious sensory issues with him, but I'd like to see the things in him which maybe aren't so obvious. Perhaps things I've taken for granted.

If anyone could steer me in the right direction, it would be much appreciated.

Try googling - I've come across several before so they're out there, but I don't remember the specific sites.
