Typing up Asperger's traits from childhood to now?

I used the Adult Asperger's Assessment to type up my traits.

You complete the AQ and EQ on the spreadsheet tabs and then on the main tab it adds those traits beneath each diagnostic descriptor.

It's still set up for DSM IV - but was pretty useful for putting together where it fitted me in layman terms.

Diagnosed with:
Moderate Hearing Loss in 2002.
Autism Spectrum Disorder in August 2015.
ADHD diagnosed in July 2016

Also "probable" dyspraxia/DCD and dyslexia.

Plus a smattering of mental health problems that have now been mostly resolved.
