What medications have you been on, what kind of drug was it, what dose were you on, and what side effects did you have on it, and what side effects did you have going off it?
What mental health disorders are you diagnosed with?
When and where and why and how many times have you been hospitalized, if at all?
What significant things did you do as a result of your mental disorder(s)?
What doctors have you had for your mental health, what kind of doctor were they, where were they, what did they diagnose you with, and what did they prescribe you?

I've been on:

Seroquel (quetiapine fumarate), antipsychotic-- 25 or 50mg at bedtime as needed-- made me too sleepy; no side effects going off it
Celexa (citalopram hydrobromide), SSRI antidepressant-- 20mg/day, then 30mg/day, then 40mg/day--made me giddy, but that was a good thing; going off it I vomited a few times and had brain zaps and the illusion of bugs crawling under my skin.
Haldol (haloperidol), antipsychotic-- 5mg/day, then 10mg/day, I think--gave me akathesia, made me yawn all the time, worsened my symptoms at first, made me forget where I was and think I was in the middle of the ocean trying to swim bt getting tired; no side effects going off it
Prozac (fluoxetine hydochloride), SSRI antidepressant-- 20mg/day, then 40mg/day, then 20, then 30-- no known side effects on it or going off it
Zyprexa (olanzapine), antipsychotic-- 10/day, then 5, then 15-- made me eat all day long and gain 16 pounds in 2 or 3 weeks, made me sleep 14-16 hours a day; no side effects going off it
Geodon (ziprasidone hydrochloride), antipsychotic-- 120mg to 160 per day, I think-- stopped working, when they upped the dose it gave me akathesia; no side effects going off it
Abilify (aripiprazole), antipsychotic-- first 15/day, then 10/day, then 20-- gave me mild akathesia the first time I was on it; no withdrawal symptoms
Risperdal (rispiridone), antipsychotic-- 3mg/day, then 1, then 2, then 3, then 4, then 3, then 4/day, then 2 as needed-- made me too tired and unmotivated; no withdrawal symptoms
Ativan (lorazepam), anti-anxiety drug/sedative-- 1mg as needed (and I think once or twice or 3 times I was given 5, and once I was given 2)-- made me sleepy sometimes; wasn't on it regularly so no withdrawal symptoms
Loxapine succinate, antipsychotic-- 10mg, then 15-- no known side effects; still on it

I've been diagnosed with:

Asperger's Syndrome (1 time)
Bipolar I Disorder (5 times)
Social Anxiety Disorder (2 times)
Depression with psychotic features (1)
Borderline Personality Disorder as an Axis II diagnosis (1)
Schizoaffective Disorder (1)

Hospitalizations:

Harris County Psychiatric Center, April 9-11th or 12th, 2008
NeuroPsychiatric Center (Crisis Stabilization Unit), part of Ben Taub General Hospital, 5 days in May or June 2008
Harris County Psychiatric Center, August 5-12, 2008
Burnaby Hospital psych ward, 7 1/2 days from the end of February to the beginning of March, 2009

Crazy stuff I did (or rather, people said it was crazy enough to tell a shrink about, but it wasn't that crazy):

I grabbed the steering wheel of the truck my boyfriend was driving and almost steered it off a cliff, I did minor damages to a hotel room (picked up the toilet and laid it on its side but then out it back, poured a glass of piss into the radiator), tried to get a friend to drink a bottle of urine (I said it was apple juice), went door to door with a fake pledge sheet for a fake charity collecting money, went door to door with a real pledge sheet for a real charity collecting money and keeping it, tied my hair in knots and put paper clips and bobby pins and elastic bands in it, got mad at people for things they hadn't done yet, got stuck in black holes in my head and became unresponsive until I got out of them and hallucinated black holes occasionally, thought people would sense a weakness in me as I walked down the street and hurt me (or rather extremist-like, thinking they would see I had a mental deficiency and shoot me), went on $400 spending sprees, and other things I forget.

Mental health doctors I've had:

Dr. Larson, general practitioner, Montreal Children's Hospital adolescent clinic, said I had AS but it wasn't an official diagnosis
Dr. Eric Frombonne, Montreal Children's Hospital mental health building, diagnosed me with Asperger's Syndrome
Dr. Scott Duncan, psychologist, University of Alberta Hospital
Dr. Douglas Roy Ginter, psychiatrist, University of Alberta Hospital, prescribed me Celexa
(Unknown name), psychiatrist, NeuroPsychiatric Center, diagnosed me with Bipolar I Disorder
Dr. Victor Cardenas, psychiatrist, Harris County Psychiatric Center, diagnosed me with Bipolar I Disorder, prescribed me Prozac and Haldol
Dr. Desjardins, psychiatrist, Mental Health/Mental Retardation Authority mobile crisis outreach team, diagnosed me with Bipolar I Disorder, prescribed me Zyprexa
(Name unknown), NeuroPsychiatric Center Crisis Stabilization Unit, diagnosed me with depression with psychotic features and Bipolar I Disorder and Social Anxiety Disorder, prescribed me Geodon and Prozac
Dr. Scardino, psychiatrist, Mental Health/Mental Retardation Authority Ripley Clinic, prescribed me Zyprexa, Prozac, Geodon
Dr. Roberto Flores, psychiatrist, Mental Health/Mental Retardation Authority Ripley Clinic, prescribed me Geodon, Abilify, Prozac, Celexa, Risperdal
(Unknown name), psychiatrist, NeuroPsychiatric Center, prescribed me Risperdal
Dr. Svetlana Malkina, psychiatrist, Harris County Psychiatric Center, diagnosed me with Bipolar I Disorder and Social Anxiety Disorder and Borderline Personality Disorder as an Axis II diagnosis, prescribed me Haldol and Celexa
(Unknown name), psychiatrist, Burnaby Hospital
Dr. Szening Yuen, psychiatrist, Burnaby Hospital psych ward
Dr. John Ronsley, psychiatrist, Burnaby Hospital, prescribed me loxapine
Dr. Bond, psychologist, Burnaby Hospital

Whoa. That's a lot.

I've never been hospitalized and I wasn't allowed to take medication, so here are my diagnoses:
Severe Depression
Gender Identity Disorder
OCD
ADD

I definitely don't have OCD though. And I doubt I have ADD.

What medications have you been on, at what dose, and what side effects did you have on it, and what side effects did you have going off it?
Prozac for a couple months back in high school, that's it. Side effects, it made me much more reactive to touch. Somebody would brush against me and I'd want to stab them. (But I didn't.)
What mental health disorders are you diagnosed with?
Depression, ADD, Aspergers
When and where and why and how many times have you been hospitalized, if at all?
Never for anything mental
What significant things did you do as a result of your mental disorder(s)?
I didn't really do anything, it was more being shunned by other kids and not being able to figure out why.
What doctors have you had for your mental health, what kind of doctor were they, where were they, what did they diagnose you with, and what did they prescribe you?
A psychologist and 2 psychiatrists, and they did little. The last one diagnosed the me, the earlier ones (in the early 90's) just called me depressed.

None. My doctor has trying to get me on anti-depressants though.
What mental health disorders are you diagnosed with?
Severe depression, social anxiety disorder, Asperger's Syndrome.
When and where and why and how many times have you been hospitalized, if at all?
None
What significant things did you do as a result of your mental disorder(s)?
For the depression I actually became depressed. For social anxiety I did a lot of cognitive behavioral therapy. I'm still working out what to do with Asperger's..positive thinking?
What doctors have you had for your mental health, what kind of doctor were they, where were they, what did they diagnose you with, and what did they prescribe you?
A councilor and a psychologist. I still see the psychologist who doesn't think medication will help me.

Well obviously you'll still have Asperger's whether you're a negative or positive person, but if you don't get so down about it you can actually start to enjoy life. I used to focus more on what I couldn't do, rather than what I can. Now I'm half thinking negative, half thinking positive, but I'm not as bad as before.

I can't remember ALL the meds I have been on but here we go for the ones I remember!
Lithium- made my hair fall out
Lamictal- made me break out in hives
Abilify/Geodon together made me have seizures
Geodon by itself caused uncontrollable arm movements and a super fast heart rate (130-150 bpm resting)
Invega caused a tumor on my pituitary gland, breast leaking, and period to stop
Risperdal caused my period to stop
Moban caused my hallucinations to increase and get more paranoid
Seroquel made me too tired (and that was a low dose)
I don't remember the rest, I know I've been on a ton of anti depressants and anti anxiety ones as well, but that was a long time ago

What I have been diagnosed with

Autism obviously
Schizoaffective disorder
Dissociative Identity Disorder
Generalized Anxiety Disorder
Depression
Bipolar (basically schizoaffective bipolar type)

Hospitalizations

1996 for a week don't ask me the dates its impossible to remember!
2006 June something for a week and October for a week
2008 February 19 (my birthday, nice birthday present!)- feb 26
twice this year but they didn't actually accept me (jan 19th and feb 23rd)

Crazy stuff I did

I move around in weird patterns sometimes when I'm outside so a laser satellite won't shoot me (now I realize that doesn't matter, they can see through your house because they fired once and missed by one inch!)
I go catatonic in public
I "lose" my ability to speak properly and start speaking gibberish and think I am a bird and can fly or my speech would become severely disorganized

Doctors I've had

Dr. Evil psychiatrist always insists that I am psychotic
Dr. Half Evil Half Good psychologist sometimes says that I am psychotic and having delusions but other times thinks its due to the extreme stress of being autistic
Case worker Basically at first said I wasn't psychotic and then she said after awhile I am psychotic.

What medications have you been on, what kind of drug was it, what dose were you on, and what side effects did you have on it, and what side effects did you have going off it?
Prozac - 5 mg a day, had major hair loss from it.
Zoloft - So far 25 mg a day... suffering from insomnia again now.

What mental health disorders are you diagnosed with?
social phobia, generalized anxiety, selective mutism, severe depression, and awaiting diagnosis of on the autism spectrum...

When and where and why and how many times have you been hospitalized, if at all?
None

What significant things did you do as a result of your mental disorder(s)?
I don't know. Leave school?

What doctors have you had for your mental health, what kind of doctor were they, where were they, what did they diagnose you with, and what did they prescribe you?
A psychiatrist, and a psychologist but only seen her once. Going to get evaluated by school psychologist eventually...

WAS prescribed ritalin, but never took it.
CYMBALTA-- ANTI DEPRESSION SSRI, 60mg, odd feelings, dizziness, etc... Going off of it was WORSE, but I moved it up so I would have most of the bad effects in my sleep. I got off of it within a few weeks on it, due to bad press, etc...

What mental health disorders are you diagnosed with?

ADHD
DEPRESSION
(I believe I have AS)

When and where and why and how many times have you been hospitalized, if at all?

For these things, I was NEVER hospitalized.

What significant things did you do as a result of your mental disorder(s)?

GUESS!

BTW about YOUR responses:

Quote:
I grabbed the steering wheel of the truck my boyfriend was driving and almost steered it off a cliff

Many, myself included, had those kinds of ideas.

Quote:
I did minor damages to a hotel room (picked up the toilet and laid it on its side but then out it back

Hey, I felt like THAT also!

Quote:
poured a glass of piss into the radiator)

Well, if it was low on water, it seems that might help!

Quote:
tried to get a friend to drink a bottle of urine (I said it was apple juice)

A number of kids try that also! People tried that on me at least TWICE! Two problems! 1. I would never fall for it anyway. 2. They have a problem with custody! My preferences demand that I be aware of that.

The theft and spending you talk about are all too common.

Was on Cipramil; can't remember the dose. Side effects were mild; just some nausea and restlessness and brain fog for the first couple of weeks.
Then was on Luvox, 100mg. This caused more nausea than the other one, as well as light-headedness and sending my stims through the roof. I'm meant to still be on it now, but I keep losing the prescription (and anyway, I feel fine). Side effects of going off it included mood swings (I felt like I was 13 again), dizziness, light-headedness, and generally feeling like I was drunk.

Quote:
What mental health disorders are you diagnosed with?

First diagnosed with depression.
Then Asperger's syndrome was suggested.
Then social phobia, which has since been undiagnosed (I managed to get my psychiatrist to understand the difference between fearing something and just not getting any enjoyment out of it).
Then,
"Social... uh... generalised... anxiety... uh... whatever it is" (quoting my psychiatrist here. It's generalised anxiety; the fear of making mistakes translates into all situations, including social ones.)
Then generalised anxiety.
Quote:
When and where and why and how many times have you been hospitalized, if at all?

Never.
Quote:
What significant things did you do as a result of your mental disorder(s)?

The most significant thing was that I failed a semester at uni (and, despite this, I didn't take any extra time to graduate :) ).
I have a lot of self-harm scars, but they don't really impact on my life, so I don't know that that can be considered significant.
I made a few half-hearted suicide attempts.
My existential crisis, and the resolution of such, came in the wake of my worst depression.
Quote:
What doctors have you had for your mental health, what kind of doctor were they, where were they, what did they diagnose you with, and what did they prescribe you?

My GP, Dr Greg Reed: diagnosed the depression, social phobia and suggested the AS, and prescribed both antidepressants.
My psychiatrist: Agreed with the depression and social phobia, then disagreed with the social phobia diagnosis, diagnosed generalised anxiety disorder; currently he and I are working on issues relating to social retardation, need for routine that makes it difficult to get anything else done, and executive dysfunction.

Ritalyn, buspar (no longer), resperidone.
What mental health disorders are you diagnosed with?
AS, ADHD, although there could very well be more...
When and where and why and how many times have you been hospitalized, if at all?
Once, for what they called anger managment (it wasn't anger, it was meltdowns, they were just the aggressive sort.).
What significant things did you do as a result of your mental disorder(s)?
Acted like an 8 year old for almost every day for the past 10 years.
What doctors have you had for your mental health, what kind of doctor were they, where were they, what did they diagnose you with, and what did they prescribe you?
Too many too count.

I'll just put my diagnoses and my past/present medications.

Diagnoses:
-Obsessive-Compulsive Disorder (pure obsessional type); five years ago
-panic attacks not associated with panic disorder or agoraphobia; five years ago
-Asperger's Syndrome; three years ago

Past medications:
-Zoloft; SSRI; February 2003 to September 2004 and then again from January 2005 to September 2005; started on 25 mg (lowest dosage) and eventually went up to 250 mg (50 mg from maximum dosage); used for "generalized anxiety" (before OCD and panic attack diagnoses), but wasn't effective and made me incredibly tired
-Lexapro; SSRI; September 2004 to January 2005; 10 mg; used for OCD/panic attacks/anxiety, but wasn't effective and made me MORE anxious and incredibly tired
-Ativan; benzodiazepine; May 2005 to September 2005; forget the dosage (I rarely used it), but it was the smallest dosage, I think; used for panic attacks, but wasn't effective
-Klonopin; benzodiazepine; September 2005 to present; used for panic attacks; dosage either 0.125 mg wafers or 0.5 mg tablets; was told to take a 0.5 mg tablet every day for a few months, but made me too tired (the 0.125 mg ones make me tired, too), so now, I only use the 0.125 mg wafers for bad panic attacks and the 0.5 mg tablets for extreme/unbearable anxiety
-Prozac; SSRI; September 2005 to May 2006; started out on 20 mg, and eventually increased to 40 mg; used for OCD/panic attacks/anxiety, but wasn't effective and made me incredibly tired
-Lamictal; mood stabilizer; November 2005 to present; 100 mg; used for Asperger's mood swings/anger/irritability; was prescribed it for what was thought to be possible cyclothymia, but realized that it treated AS symptoms after AS diagnosis
-Niravam (form of Xanax); benzodiazepine; May 2008; forget dosage (only used it once)- it was prescribed to try and find a benzo that didn't make me as tired as the Klonopin, but this made me MORE tired/out-of-it than the Klonopin
-Abilify; atypical antipsychotic; June 2008 to July 2008; started on 5 mg, increased to 10 mg; used for OCD/panic attacks/anxiety, but wasn't effective
-Anafranil; first SSRI used for the treatment of OCD; July 2008 to present; 50 mg (next-to-lowest dosage); used for OCD/panic attacks/anxiety

I think that's all of the meds I've been on. Just as a sidenote, Anafranil is my miracle drug! I rarely have obsessions anymore, and it's even helped prevent my panic attacks! I'd say that I get panic attacks 65% less of the time. And I noticed a complete change in only four days! :D It's rarely used anymore, except in cases of treatment-resistant OCD like mine, because it has more side effects than the newer SSRIs (like Zoloft), but I haven't had much of a problem with side effects. I get dry mouth a lot (the most common side effect), but I'd rather deal with that than obsessions!

Anxiety, social anxiety, depression, and PDD-NOS. Possible AS.
Zoloft 50 mg but increasing to 75 mg. Not really any side effects on it. Without ti I am irritable, depressed, have more sensory issues, more anxious.

Self suspecting that I have ADD/ADHD.
