On the NHS bmi calculator it says when you hit a body mass index of 28 specifically that's when you are at higher risk of health issues such as diabetes and heart disease etc.

If you have a bmi of 28 or over but its mostly muscle would that excess weight still cause health issues such as heart disease and diabetes?

It depends on the person, really.

One person might have a "good" 28 BMI; another a "bad" 28 BMI.

What do you mean by "good" bmi 28?

I am guessing your heart does not care if your excess weight is due to fat or muscle

It’s better if the excess weight is muscle.

Yes that's true.

This can be plausible. But people with 28 BMI and relatively low bodyfat are very rare. You'd need to have at least 2 out of these 3 things: regularly bodybuild for 5-10 years, steroids, god-tier muscle-building genetics.

Even many bodybuilders who train for years might barely be overweight (i.e. 25.1).

A lot of bigger guys overestimate their muscle mass and have more to lose than they think they do.

Here is my lifestyle at the moment

During the week I drink loads of water and eat a healthy diet, walk 10,000 steps a day and lift weights 3-4 times a week. However usually on Friday and Saturday socially I drink beer with friends.

I currently weight 162 pounds without clothes on in the morning which for my height gives me a bmi of roughly 24.9.

Do you think I need to lose some weight or not?

24.9 seems healthy. I wouldn't say you need to lose weight. If you're motivated by attaining fitness goals or achieving a lean aesthetic maybe. But 24.9 seems healthy, moreso than most people in developed countries.

24.9 is right on the upper end of the healthy bmi range though. Just 1 or 2 pounds more and I would be overweight.

Also what do you mean by “if your motivated by attaining fitness goals or achieving a lean aesthetic?”
