I never received a report. The psych team (psychiatrist, psychologist, nurse) obviously wrote a report that is in my file. They sent a one-liner to my doctor. They wrote a one-page report confirming the diagnosis for a different organisation that provides support for adults with ASD. I have copies that I can use if I need to "prove" the diagnosis, e.g. for welfare claims.

Private physicians are entitled to charge for their time, so yes you pay a consultation fee and a letter-writing fee to get further paperwork - it is economic to get a full report yourself that you can use, rather than paying the private physician to write to each person who needs a copy. I have been charged 200 to 400 dollars for professional opinions (excluding the original assessment), and just the consultation fee (60 dollars) by my doctor.
