How do I let someone know when something bothers me?

Hi:
In the past I have often come off too strong when communicating with others that something bothers me. It have usually gotten upset and said a little too much. I would like to speak to someone without using such as "I'm mad at you."

It's better to say you're upset at what a person does, rather than the person.

I am also bad with this myself, but a little different I think.

Usually I'll do whatever I can to avoid a situation becoming worse or an argument developing. The times I've try telling the other person/people I'm unhappy, I'll spend a little time in my head beforehand working on what I want to say, all structured in a logical order, so it makes sense in my head and expressed in a way not likely to get them offended, most of the time not having any luck there. I've had so many backlashes my many different people, most of the time I end up retreating from the situation or isolating to get away from them.
