Is that down to a lack of affective or cognitive empathy?

Some comments from my sister.

Tim was, to be hones, quite unable to think of others when he was in one of his bad phases. By that I mean he had no real understanding or true empathy around the impact he was having on those around him,

Tim certainly has compassion and kindness but still struggles I think with empathy on occasion.

To me, it's cognitive, especially the latter - "compassion and kindness" are signs of affective empathy.
