Personally, one of the all-time worsts was when Disney nullified the Star Wars Expanded Universe (now "Legends").

It doesn't help that the Sequel Trilogy (the EU's replacement) has been even more infamous than the Prequels.

If multiverse theory has ever taught me anything is that the expanded Star Wars universe can still be canon if you want it to especially if Disney's official Star Wars canon is less than stellar.

For the "official canon", Universe A: Star Wars trilogy, prequel trilogy, Disney trilogy, Disney spinoffs, CGI Clone Wars and Rebels series.

For me, Universe B: Star Wars trilogy, prequel trilogy, 2003 Clone Wars shorts, Clone Wars video game developed by Pandemic Studios.

For you, Universe C: Star Wars trilogy, Prequel trilogy, any Star Wars cartoon, video game, novel, or comic you wish.

For Lego fans, Universe L: Lego Star Wars video games and animated films and shorts.

For no one, Universe F: Star Wars Holiday Special and Ewoks cartoon are added to the canon.

P.S.: I actually liked Rebels and Rogue One, but those were exceptions to the unfortunate norm.

The Simpsons has been on the air for more than 30 years. I've only watched up to season 17. I'm pretty sure somewhere within the writing's lack of new and good ideas there's more than a handful of episodes that retconned some well established backstories and pissed off many Simpsons fans. Multiverse theory can't save this one. If the writers really plan on ending the show by having the plot loop back to the first episode, Multiverse theory can't save it.

I don't know if this counts but I hate it when actors leave and instead of talking about the characters, they just act as if they never existed in the first place. Really?

I was watching a video a while back that talked about these sorts of things, with worst examples. There were things like a character, in I think Happy Days or something, who went up stairs in one episode and was never mentioned again.
