Hi:
There was an acquaintance who I used to associate with who has a diagnosis of PDD NOS and other related processing disorders. That said, this person has extreme black and white thinking and will tell you exactly what is on their mind in a very negative light. They also have no filter or any sense of boundaries. It's so severe that other people often get mad at them.

For example, someone will post a nice comment on social media only to have this person completely ignore the purpose of the post. Instead, they always find something negative. When people get mad at them, they don't seem to understand why.

Other characteristics
1. They are controlling
2. They throw tantrums when they don't get what they want or things don't go their way

A personality disorder sounds likely.

I wouldn't say that would be an "underlying" issue though, more like the other way around, since personality is what develops on top of more definite and innate characteristics. Predisposition is a factor in all personality disorders, but rarely if ever the only factor.
