and I was a Math major (double with Computer Science) in college.

It's always felt like my living situation and resulting lack of sleep was destroying my cognitive functions, and having spectacularly failed a (very simple!) math exam that was part of an interview process (to get a job in a place with cheaper housing so that I can ****ing sleep at night), I'm worried even more.

This year has been a nightmare that seemingly will never end.

I want to die.

I'm sure it's not permanent. You just sound exhausted.
I've got nothing to help you break out of that catch 22 though.

its common to have difficulty with simple math after having studied it at higher levels (at least thats what all my GRE study books say)

Was it that you couldn't remember how to do the math or that you were just too nervous or tired to do it. If its the first case, there are a lot of step-by-step guides out there that will help you through and you just need to refamiliarize yourself
