This is the part that always gets me stumped...whether I'm writing a story, a script, a song, making a level for a game...I'll always have tons of great details for an opening, or a particular scene, or a room in a level...then I never have any idea where to go from there...I figure it's part of the "paying attention to details while being unable to focus on the big picture"
