| Apollo one (January 27, 1967) failed on the launch pad and incinerated 3 astronauts. Apollo 13 (April 13, 1970) was, in the words of NASA, a "successful failure" in that the astronauts were able to learn survival workarounds despite the damage caused by that exploding O2 tank. |
