Is there any mention about a proper exercise going with the diet? So he doesn't understand dry humor, he doesn't understand vague suggestions, or blanket statements. It has to be concrete. If I try to even use a saying like "I have eyes in the back of my head" he doesn't get it, he will say I'm lying. These are excellent examples of pragmatic issues. So...
