The only time I see it as a problem is when you cannot go out of the house or be seen by other people and be comfortable while doing so without it on. I normally don't think about anything actually. I find it quite difficult to get off if my mind is preoccupied with other matters, and even if I do manage one, the orgasm just doesn't feel as good.
