It's not really an accident - the "iterative design program" is all about "building fast, testing, finding failures and fixing them". So, some failures in the current Starship project certainly have been found, awaiting fixes.

That's what I did to my old car. I did a "rapid unscheduled disassembly" of it, and another car. Saying "you totaled two cars in an accident" is such an unpleasant way to put it! :D

if i ever did a rapid unscheduled disassembly of my dishwashing machine at work it would probably lead to me recieving an involuntary permanent vacation immediately.

In fiction, Rapid Unscheduled Dissembly can be staved off by Plot Armour, or the Structural Integrity Field as Star Trek: Voyager called it.
