Its the misguided concept that the people of a totalitarian regime have any control over their government or its actions. The "dogs of war" concept is much more effective. Fast, efficient regime change, leading to an adjustment in local and then national policy. There is no real reason to carpet bomb whole areas. (Though to be fair, you have to wonder about the people who use civilians as meat-shields in the first place. Which is lower?)
