email notifications on threads not working?

When I click the "Notify me when a reply is posted" I don't get emails when someone replies to the thread. Sometimes I get a notification for an old reply that i've already seen and replied to (or posted after that reply was posted).

This problem is relatively recent.


Please fix it, if possible.
