Starting Community College In Sept

I'm currently enrolling in school after years of repeatedly not getting good grades, and struggling to keep my GPA up. I often find myself confident as I go into start the semester, but as the semester continues, my nervousness and anxiety end up taking over, which results in my not finishing or getting a low GPA at the end of the semester. I want to go back to school and actually finish college to get my degree.

What is some advice if you are in college or getting ready to start college that you could offer that might help me with these issues and actually succeed in college (this time) ?
