I've had a few factory jobs, but no injuries, just some near-misses.
Due to sleep deprivation while operating the mulch bagger I dropped the chute and nearly severed my finger.
I say nearly because I succeeded at my reflex check and took zero damage (yanked my hand out of the way at the last split-second), but my glove got stuck in the machine.
Changes were made to the process as a result to ensure that no one else could repeat my incident.
Multiple jobs I've had (both manual and more intellectual) have gradually resulted in me becoming increasingly unhinged due to bipolar symptoms. Not sure if that counts though.
