Hi all thanks for the wonderful replies, i just thought I would update you all.
As I said in my first post J has other medical issues as well as the AS dx. Last week I met again with one of J's original doc's from years ago, I explained what was going on with J's underwear. After careful examination he then explained to me (something he should have done eight years ago) J doesnt have a normal urethra (not sure if I spelled that right), J's is surgeon made ie it was constructed out of skin. 
The muscles surrounding his urethra dont work as well if at all as a normal one would, there for urin is still sitting in the urinary tract, hence additional irratation. He also believes that some of the problems are sensory caused by the AS. So the up shot is J got a double whammy! 
He has told me to give him salt water baths a few times a week, he has shown us how to try to get rid of any excess urine. It looks like the best I can hope for is to minimize J's discomfort, he still has sensory issues along with everything else.
I am annoyed to say the least, that this all wasn't explained to me years ago. For more than 8 years now my poor son has had to put up with all this extra irritation. I can only imagin how he must be feeling, the sesnsory issues alone would drive me nuts.
