The scariest thing for me without a doubt would be other players. There are some genuinely scary people out there.

The scariest thing that I ran into would have to be the monsters from Amnesia: the Dark decent and Penumbra: Black plague.

The adventure game 5 Days a Stranger had some pretty scary stuff about 1/2 of the way through and the atmosphere keeps building from there; I remember being spooked by some of the scenes, which is impressive considering it is a point and click adventure game. If you check it out, be sure to have the full sound and music on, as that really added to the atmosphere.
7 Days a Skeptic (the first game's sequel) also was somewhat scary, although is more like the movie Alien.

I also found the intro room for Alone in the dark to be pretty scary. At the time I tried it (several years ago), I couldn't get my computer to get past the first room, from a technical standpoint, as I couldn't get the equipment to work and got eaten by monsters. Based on that (as surely the actual game is even worse), it was probably even scarier than the other games I quoted.

Also, there was some zombie/alien invasion survival 3D adventure game that I thought was quite scary and liked playing, but couldn't play more than 20 minutes without getting motion sickness; I'd recommend it, but can't remember the name as I didn't get far enough through it to learn much of the plot (it might have been Half Life).
Edit: the game I was trying to remember the name of is "Singularity." It was only the first part of the game that I played though, due to motion sickness, so it might not be scary later in the game. Mass Effect 2 - Human Reaper Larva:

It terrified me. At first it seemed nothing more than a blatant rip-off of the T-800 endoskeleton from the Terminator films. But after destroying the injector tanks, to my dismay, the Reaper baby was far from "terminated". I think it was the weird mechanized groans it made as it moved ropily about the platforms that got to me the most. Ugh.

Mass Effect - First Encounter with 'Sovereign':

When on Virmire for the base assault mission, you come across a hologram of the Reaper known as Sovereign, Saren's puppeteer. The creepy voice, so cold and unaffected, sent chills down my spine. I've never been made to feel so insignificant compared to something so unfathomable in a game before, but Mass Effect managed to render this sensation to great effect.

Honourable Mention - Lisa - Resident Evil Remake for the GameCube:

She had a tragic backstory that really made me pity her, but she was also a ravenous monster that had knocked me out on one occasion, and tried to kill me on another, so finally killing her felt like I'd done her a service. The strange moan she would utter was also nails-on-chalkboard disturbing. Do not want!

First thing that came to mind was the Regenerators in Resident Evil 4, and the scissor man in Clock Tower.

Crimson Heads - Resident Evil Remake (GameCube):

Remember that zombie you downed a few rooms ago? Yeah, you see, in Resident Evil's remake for the GameCube, there's a good chance that unless you burned or beheaded that thing, it's going to come back faster and stronger than ever.

And it will be blood red, hence the name "Crimson Head". With elongated claws to tear you to ribbons. 8O

I both feared and hated Crimson Heads because they were so swift and powerful, but upon replaying the game a number of times, it becomes possible to ascertain which zombies are likely to undergo the re-revival that precipitates the "birth" of a Crimson Head. :wink:

Anyone played Dante`s Inferno? You fight versus mutilated baby corpses, gigantic dead fat people that try to eat you, etcetera

I played the game once and finished it, then locked it away to never be seen again.

The first time I was genuinely freaked out by a video game character was the first time I encountered a fast zombie in Half-Life 2. I hate those guys.

Doom scared me when I was younger and then... there was Doom 3. That crap is scary. Sadly I barely played silent hill, so I've missed a lot on that. Maybe I should put the HD collection on my list. Probably won't get to it for a while, though.
