Was it Bonk's Adventure?

It originally came out for the TurboGrafx-16, and was also on the NES and SNES.

"hits people with your head" definitely sounds like Bonk.

I've actaually never heard of either of these games, but judging by (A) your dicription and (b) this clip I just found on youtube I'd say the other respondent(s) who said the game is bonk is(are) right

To continue the prehistoric island theme, were Hudson's Adventure Island and Wonder Boy really the same game?

At first they were the same game (Hudson changed the title for legal reasons), but for the sequels, Adventure Island tried to reuse the same formula for a while, while Wonder Boy evolved into something completely different.
