I think for me it was Mimi in Super Paper Mario when she turns into that spider-looking thing and screams "MIMIMIMIMIMIMIMIMIMIMIMI!!"

Alessa or Dark Alessa from Silent Hill,

Though I guess technically her child or God is the antagonist depending on how you look at it... Damn freaky game.

I thought the Big Daddies from the first Bioshock were pretty creepy.

Can't remember what the game was, but there's a final boss on some game where it's supposed to be some kind of unborn fetus.

Yeah, that's the one. Such a disturbing thing. I don't know if it's technically a villain, though.

It sure looks villain like to me. Maybe it's some sort of inner or outer darkness come to life?

Yeah. Definitely would appear so. The only reason I say it may not be a villain is because (from what I remember, anyway) I think it was technically being forced to act as a villain for whatever reason.

Andrew Ryan is a well written villain, the Bioshock games provide some of the best writing and atmosphere that can be experienced in a game.

Sovereign from the first Mass Effect deserves a mention, too bad they went Matrix revolutions with Mass Effect 3.
