In some cases the perpetrators are also romantic partners. There's often factors that complicate things, it's not as simple as you're suggesting it is; they're not all literally the result of rape.
If one is a voluntary participant but is also in a situation where their ability to actually not comply is limited they may or may not view that situation as rape but it's still fair to describe their consent as partial and coerced. When I had to f**k someone I didn't want to f**k to keep a roof over my head it wouldn't be fair to describe that situation as me being raped, I agreed but the other option was to no longer have somewhere to live. It's not something that's 100% binary even though we might like it to be.
In situations where women have reduced access to housing, employment and education this will reduce their ability to be independent and people who are dependent are vulnerable to coercion, this can extend to being coerced into sexual activity.
In places where access to birth control is limited unplanned and unwanted pregnancies are more likely to occur. It's biology, not spite.
