Reagan was diagnosed with Alzheimer’s five years after he left office. When he was in office, his doctors gave him a clean bill of health.
During his re-election campaign he was widely perceived (including by his son) as having bumbled in his debate with Walter Mondale, which was the beginning of the public speculation about whether he might be going senile. Of course he still won the election.

People don’t appreciate how severe clinical dementia is. Yes, you can still function in society with dementia, but there is a long way to fall between the mental performance of the average 65 year old and the mental performance of someone in the early stages of dementia.
Reagan got through 84-88 just fine. No reason to think Biden couldn’t manage it as well.
