That's nothing. about 20-25 years ago there was a incident in orange county, CA where a cop accidentally blew another cop's head off in a unloaded training exercise. Both of them are guilty of not checking the chamber. Unfortunately, that part of my google-fu is weak.

Cops qualifications in firearms proficiency tend to be grossly overrated. They are held to a much lower standard of liability for poor handling and use of them than the public is.
