Yes, this is extremely common. What are referred to as "unmonitored live feedings" are highly problematic in reptile husbandry.
Ignorant reptile owners don't understand that feeding in captivity does not imitate feeding in the wild. In the wild, there are no walls, the snake only pursues food when it is hungry, and both animals have the chance to get away.
This is why feeding frozen, thawed food is the preferred feeding method. It is really no different from feeding a dog or cat, or eating meat yourself, except that the reptile gets a whole animal that has not been cooked.
Live food should only be offered to reptiles that will not eat otherwise. This should be done under careful supervision, with the prey item being removed if the reptile does not show immediate and enthusiastic interest.
