You really need a social worker to help you apply for various programs. It's really the only way I think you could improve your situation.

I don't think it's been mentioned yet, but besides housing and other programs you'd qualify for, I think you're eligible for a free phone provided by the government. My husband has one, and he's on SSDI. It includes unlimited texts and 500 minutes a month.

If you make it through the hearing at your shelter and can stay there for a while longer, maybe you could use the shelter's address.

These phones are extremely basic - much simpler than a regular cell phone. If you can post here, you can learn how to use an Obama phone.

Regarding the care home definition, here is the type of home that I am speaking of:

"Supportive Housing Programs provide an alternative living arrangement for individuals who, because of age, disability, substance abuse, mental illness, chronic homelessness or other circumstances, are unable to live independently without care, supervision and/or support to help them in the activities of daily living; or who need access to case management, housing support, vocational, employment and other services to transition to independent living."

It really baffles me how you haven't been referred to one yet, as you meet many of the qualifiers. I think you need to find a social worker but I'm not sure how that is done. Still, we have to keep trying. :)

I don't know how I missed it, but why is there now a hearing at the shelter?

Apparently he faces a potential 3-month ban from the shelter, due to health issues. And it seems to me, if a person's health issues are too severe for shelter living, the streets are an even worse idea, and a care home would be more appropriate.

Hopefully his HOT team will guide him toward the next step. I found this info page which seems to indicate the HOT team can in fact point him toward whatever community resources are available. (It's true they can't make the official decisions and approvals, but their job is to point him toward the agencies that can help him):

http://sfhomeless.wikia.com/wiki/Homele ... .O.T._Team

I tried to figure out the process involved in getting approved for Supportive Housing, and this seems to be the program in charge of it:

https://www.sfdph.org/dph/comupg/oprogr ... igible.asp

It sounds like he would be put in an SRO situation, similar to what he had before. (Which makes me think he did have a case worker at one point, that got him into that one?)

And unfortunately according to this FAQ, a previous eviction would make him ineligible unless his case manager can prove that the problem has been resolved since then. (Which it hasn't, since it's an ongoing health issue.)

https://www.sfdph.org/dph/files/DAHfile ... 122013.pdf

So it seems his chronic health problems put him at risk for being ineligible for homeless shelters and supportive housing options at this point.

In which case his options are becoming more and more limited: either (a) live on the streets, because he's not healthy enough to qualify for homeless shelters or supportive housing; or (b) accept a more-or-less permanent care home solution, that closes the door to his dream of moving out of state, attending a 4-year residential college, etc.

The official terms I've found for these options include: 'residential housing', 'board and care home', 'assisted living facility', and 'nursing home'. I'm not sure which type ASS-P would qualify for, but I would hope that a person who can't afford regular housing due to disability, and is too sick to live in a homeless shelter or supportive housing, would qualify for one of these programs.

But it does seem to be voluntary, so until ASS-P accepts that he needs a higher level of care, there's not much the HOT team can do, aside from connecting him with free meals, outpatient medical care, and whatever community programs he's capable of taking advantage of, even with severely limiting health problems.

I hope the shelter hearing goes well on Monday, and he can continue to stay there for now. If he can get his current health problems under control, there is hope he could qualify for a supportive housing situation again in the future, whether in SF, or Nashville, or someplace else.

Speaking of Nashville, I too am concerned by DCJ's absence, and hope that he's all right. But I would guess that he won't be in a stable enough position to be helping anyone else for at least another month or so, and in any case the trip would require careful planning, and preferably for ASS-P to be in better health, so it seems the Nashville plan is on the back burner, for the time being.

Take care of yourself, ASS-P... I hope you can get these health problems under control soon!

ASS-P had a social worker, who did point him towards the
SRO he had stayed in previously.

Apparently, another social worker helped him somewhat with his college application. It was for a community college/2 year school.

Apparently, he was thrown out of the SRO because some guy who didn't like him ratted him out about his "elimination problems."

I wish he would cooperate more. He really doesn't want an environment which is, from his viewpoint, restrictive, and would force him to take "basket weaving" classes.

That seems to be the big question here - exactly how restrictive would such a place be? Would it truly close the door to ASS-P being able to follow through with his other life goals, should his health improve in the future?

'Residential Housing' seems to be the same thing as a 'Board and Care Home'. According to this page, there are very few openings for low-income people, and it's designed more for those capable of paying the full cost:

http://www.calregistry.com/housing/bce.htm

If ASS-P is serious about getting help, it seems a Public Guardian would be the person to help advocate for him:

http://www.sfhsa.org/372.htm

ASS-P has been reluctant to accept this option in the past, as it would likely involve the Public Guardian managing his money. I think it's true that some of these care facilities (nursing homes in particular) would budget all of his SSDI money toward living expenses, leaving only $15 a month for personal spending money.

This is a reality that many of us will face at some point (I fully expect that I will) - and it's something I'd like to understand better. I think it would be helpful for ASS-P to know exactly what this entails, so that he can make an informed decision, and not one based on a vague fear of 'basket-weaving'.

So I'll be looking into that some more, but if anyone is knowledgeable about 'residential care home', 'assisted living', or 'nursing home' facilities, please enlighten us on this issue!

This is the main reason why ASS-P needs a case manager more than anything else. A case manager would be able to describe and answer any questions about what a potential (or specific) group home in the area would be like. A case manager would be able to give ASS-P some options that are available which would enable him to make an informed decision better than any of us could online. Then he would help him to get into the program by applying for him and handling all the paperwork. Sometimes these people can pull some strings when a lay person can't.

Why does he not have a worker right now??? I can't emphasize how essential this is if he does, in fact, want to improve his situation.

My husband has a case manager, so I know from experience that it does (and would) make all the difference in the world. They would let him know everything he is eligible for such as housing, food stamps, a cell phone, and other community resources.

Maybe we should limit ourselves to helping him find a case manager because everything else he would need would follow.

My husband has a case worker for free because of his disability. I'm sure ASS-P would qualify as well.

Does ASS-P have Medicaid?

There are so many resources available for people with disabilities...

Your fears are most likely ungrounded. You need a case manager who can answer all your questions about a specific housing situation. Then you could decide whether or not you want to do it. You'll never know for sure unless you get the facts.

I'm sure there are different gradations of group homes. Some probably allow more freedom than others.

Do you have a case manager or social worker?

If you had a worker and you were in a group home, they could help you fight back if you felt your freedom was being infringed upon.
