I try to be mindful of this and save my pragmatic advice for those who might be in a state where they'll make use of it. Sometimes the person is in too much pain for rational approaches to be viable and in those cases the first priority should be soothing them enough to get them back to being capable of considering rational and pragmatic approaches.
Sometimes it just means admitting vulnerability before giving the pragmatic advice, sometimes it's not bothering with it for the moment.
