Because I am asked a lot of questions and would like to answer them, how should I respond if someone who isn't me is having a hard time understanding me and if it is a family member or friend?

What, specifically is the problem? Enunciation? Stammering and stuttering? Thick accent?

It's more like this Fnord:

And it feels upsetting everyday when I talk like that.

Corporate firewall won't let the video play.

It's an example of disorganized speech from schizophrenia. There are no swear words or obscene content.

So you tend to jump from topic to topic with no clear pattern ... ?

If it's the result of schizophrenia, then you be better off to consult your doctor.

I do, but what if I can organize my disorganized speech more to make it sound clearer so that it does not sound so disorganized and that the thoughts being expressed from me are clearly stated.

Sometimes I wonder if I am talking not clearly enough or if ass holes are deaf or not paying attention

They keep grunting "huh" and "what" at me, like it is the etiquette equivalent of "excuse me"

Sometimes they did not hear I said something

Sometimes they mishear

I listened to the video. It is rambling speech, very disorganized and fairly nonsensical. I would refer to it as speaking off the top of your head (in an extemporaneous manner; without careful thought, preparation, or investigation).

I try and avoid that. I generally prepare well beforehand and provide a scripted response.

I generally find that speaking one on one is not difficult. But when you speak to a group of people, you need a good deal of preparation. In group speaking I generally bring along an item (like show an tell). Something that is related to the subject at hand. It is a concrete item and allows other to focus on something tangible at the beginning to draw the audience in.

You might try Toastmasters. They could help you hone the skill you desire.
