I think he meant underage to mean someone who isn't an adult. In my country, although there is no national law against piercings for younger people/kids, businesses can place restrictions on who they allow to be pierced on their premises. Some businesses allow only 16 year olds or older to have piercings.
So it isn't surprising that some people feel that there may or should be an age limit applied to who should be eligible for that sort of thing.
I'm not commenting on what he actually meant, I think we agree he didn't mean anything squicky.
I'm just explaining why it sounds kinda squicky and how it's a poor choice of wording because of implications.
You don't need to defend him or explain he didn't mean anything creepy. I know he didn't. If I thought he was intentionally being a creep I wouldn't bother trying to explain why it was a poor choice of wording.
