No movie really really scares me anymore but when I saw The Ring in the cinema when it first came out, that really scared me. The Japanese version of The Grudge is pretty scary.

The Exorcist, back when I first saw it. I'm sure the movie scared plenty of people, but for a Catholic 10 year old it was utterly terrifying.
