They're not people.
It's a review of your disabilities and what will happen if you get worse/better.
It's usually for old people but if you have a degenerative condition you can get one too.
It's completely voluntary, so you must have filled it out at some stage.

What I meant was, the characters are intended to display that death is nothing to be feared, that there is no death, only life.

Apperantly not, they seem to be rather persistent on the idea... by never leaving.

Which is ironic to me, a bit exploitive to me but they don't care.

Basically, think Death, but with wings and...um...semi-physical forms instead of bones [and cats, of course]. Oh, and the protagonist's grandmother is very curmudgeon-y and equally amusing.
