I think it basically amounts to "Do you think like an NT, and thus understand an NT?". I bet if nearly everyone in the world was autistic but 1% were NT, it would be changed to "Do you think like someone autistic, and thus understand someone autistic?" and then every NT would be labeled as having the Neurotypical Disorder, lol. (Which is a rampant disorder, I agree!)

It really seems to come down to whether or not you think like the majority, and thus understand the majority, regardless of the way the majority thought. (However, I suppose if someone lacked a "theory of mind" and wasn't just inaccurate at it (Inaccurate at understanding NTs, anyways), that would be a serious socially cognitive deficit, regardless of the way the majority thought.)

My stock answer to "how are you" is "not too bad". :D Which is an honest answer most of the time. And you can probably guess that nobody ever asked me to elaborate why I'm "not too bad" instead of "fine". They don't want to know. Once they hear "not too bad" their alert is on "oh no if I say any more she might tell me all her problems." They tend to stop right there or change topic really fast.

The funny thing is my family doctor always ask "how are you". Well if I was fine I wouldn't be seeing her, right? It seems a bit ridiculous to say "I'm fine." then go on to say that your throat is throbbing and your head is all foggy and you chest is congested, but I'm sure lots of her patients did just that. :D
