Maybe it's a working memory problem? I do well on other kinds of visual/spatial tests but I can't do rotations. I get the same frustrated, sick feeling that I get when I do other kinds of working memory tests. I just can't hold the image in mind long enough to do the rotation.

I sometimes "drop" whatever it is I'm trying to rotate in my head while I'm rotating it. :D (maybe my ADHD showing up?) It is sort of annoying because I lose my place and have to start over again.

I was absolutely terrible at this until I got into 3d modeling. After that, I can almost instantly rotate a shape in my head.

This is something that can be learned. Blender is a free 3d modeling software. If you decide to use some tutorials to make even some rudimentary shapes, after a while your brain will be able to turn things around more quickly. This is because you have to rotate your scene pretty constantly around whatever you're making. If you get into texturing by unwrapping UVs (It's kind of a drag) that's even better practice.
