Chewing noises, but not the empty soda straw bubbly thing.
Little dogs that sound like squeaky toys, like my huge Chiweeny has.
Loud music, any kind.
Thumping bass music in a car, sounds like someone beating on the trunk.
Back-round noise that covers up what somebody is saying to me.
The 150% volume of all TV commercials.
The endless clammer of someone else's TV.
People talking to me when I don't have the energy to listen them.
Any noise created by anybody just "for the fun of it".
The unconscious tapping of a pencil, clinking of a coffee spoon, etc.
People sucking their teeth.
A mosquito or gnat circling or attacking.
People that shout when they sneeze, or
People that recover from the sneeze loudly and repeated, oh, oh, ah, oh, man, oh, gee, oh....
And THIS DAMNED TINNITUS ! !!

Whistling is a sound I really kind of like unless it is really loud and done in an obnoxious manner.

Someone eating loudly i.e. chewing, 'chapping' and crunching food.
People tapping their fingers on a table loudly, whilst the whole room is quiet or you're in a written exam.

Any sound that comes from styrofoam.
Someone writing with a pencil.
Fingers or paper moving across slightly rough paper or cardboard.
Music or speech so quiet I can barely hear it.

I'm mostly bothered by quiet sounds.

Edited to add: I'm also disturbed by the sounds of planes and helicopters, though.

the reverberating bass on car stereos that seem so much in fashion these days. I wanna rip someone a new one, but it's impossible to tell which direction they're comming from. They're like bullfrogs, announcing their desire to find a mate-- pretty pathetic for human species to have to resort to... I have called every agency in town, and to hear the police/city manager/etc etc talk, I must be the only person who hates them

This, big time.

People smacking their lips together after yawning.
Stupid music and TV commercials. I swear - every song I listen to gets stuck in my head.
Little dogs
TV/radio static when it's too loud
Obnoxious, giggly teenage girls *facepalm*
This sound the dog makes with his mouth...it's like a licking sound but he's just opening and closing his mouth. I can't stand wet, sloppy sounds like this.

There's more.....

Really long metal poles (maybe like construction scaffolding?) have a horrible piercing clang. In high school paddle tennis became really popular for some reason so they let students take these long bendy metal poles out and place them on two garbage bins as the 'net' they had to hit the tennis ball over to each other. In the quadrangle there used to be four or five of these games going at once with all the poles constantly falling down. I couldn't believe I seemed like the only one who noticed how horrible it was & had to get away from it.
People drum tapping on something becomes annoying after a while.
Commercial top 40 radio :? I'm not like an arrogant nazi about it or anything, in fact I usually don't complain out loud about it but the hosts are usually vacuous air heads that think they're funny but aren't and the music is just soulless, homogenised, pasteurised garbage. I don't know if that counts but to me commercial radio is it's own unique sound of blerk!
