When someone uses my name I feel like they are taking something from me, or getting too close to me. It is similar to if someone was standing in my personal space. I just feel really uncomfortable and agitated and think "who gives you the right?!".
I also have a similar problem to GregCav - I can't pronounce my name properly, even though it is a simple and common name. For me it is wrapping my tongue around the 'r' too. I have been using a nickname for over ten years to try to get around having to say my real name anymore. It has helped.

Sometimes I get uncomfortable too when people call me by my name. If they are friendly and seem happy when they say my name then it doesn't really bother me, but if they call me by my name and they seem indifferent or unhappy that's when I feel uneasy. It makes me feel like they are judging me or trying to intimidate me. I agree, I too get that same feeling I get when someone is invading my personal space.

I've never identified with my name, and I tend to not call other people by their names either. Like others have said, it just seems improper somehow, like crossing a personal boundary.

Growing up, I'd reflexively cringe whenever someone said my name. Other kids would whinily stretch the vowels out whenever they got annoyed with me, so I began to associate my name with contempt.
