You don't really need to leave town. Everything you need to survive is right there where you are, if you access it. I am repeating myself, but if you got yourself admitted to the hospital, the Social Services department would assign a Social Worker to you. You would not have to make the effort to research available services. He or She would do that for you. They could also make sure you had a prescription for appropriate meds, and not pain killers all the time. Meds that would help you to function, and not impair you, like the ones you are currently taking.

You don't need a millionaire. Perhaps there is a group home that would take you, based upon your AS?
