Phone Conversation Memory

So, a few minutes ago, I get a phone call from the psychologist who assessed me, confirming the date and time of my follow up appointment to go over the results. After a relatively pleasant thirty second conversation where I thanked her for confirming the times and apologized for not being certain I'd gotten it down properly the first time (and generally felt quite comfortable for once), I hang up only to realize I hadn't heard a single thing she'd said. I have no idea what time the appointment was supposed to be. I had to call right back and apologize profusely on her answering machine (must have left the office?) because apparently, I can't talk on the phone and pay attention to what's being said at the same time. Unfortunately, this happens all the time and I feel like I've legitimately irritated people in the past.
Anyone else have this sort of thing happen?

I always keep a note pad ready to write appointments down straight away, and write down basics such as my own number or license plate number before the call in case I need them, because more often than not when in phone conversation I can't recall even my own number or address etc.
I have terrible short term memory to the point that in phone, spoken word, people images etc. so bad that even my own doctor made it one of my required needs to keep a notebook handy at all times for everything. I couldn't describe things from memory and use applications to alert my next diary appointments which includes daily needs ![]()
This, despite being very able to remember facts and subject matter within the long term memory.
