I'm really confused. It's not even a word, yet it seems to be an Internet staple food, in terms of conversation openers, but I don't know what it means. Ive sort of gathered it must be along the lines of 'hello'? Or a 'how are you' line, (the type you say fine, you, back to).

Or, if it's both, what do you reply? I usually pretend someone hasn't said anything when they pull out the 'sup' card, because i have no response ready, but it kinda frustrates me that I don't understand.

Anyone have a translation, or hates it when people can't use less cryptic language?

It's shorthand for "what's up!"

I think a more accurate translation would be "What have you been doing lately?". If it meant "How are you doing?"', I've never heard anyone reply "fine" to "what's up" before.

It means "whats up?"

You reply either with "nothing much" or something along those lines, or by telling them something that's happened, ie; "got a new car", "lost my job", "went to the fair" etc.

If you want to be reciprocal you can say "nothing much, what's going on with you?" or something.

I believe it can also refer to "supper" as in a meal (sometimes used as a verb). Context should probably make it clear which usage it is though.
