I'm from Wales. I received something similar called a "psychological assessment report". It details my Aspergers severaty and such. It is the validated proof that I have Aspergers. My guess is that a report like that lists all confirmed disorders. It can be used for declaring the disorder for say... employers, benefits or other people that need to know.

It's just a several page document listing the interviews and testing you did during your assessment and will go into detail about your results on each one. There will be a listing of your diagnosis and a page or two written by your pdoc explaining the reasons why he or she believes the diagnosis made falls into line with the DSM criteria. At the end there may also be a list of suggestions your pdoc has for assisting you in future with medication and/or therapy.

It's more or less the you officially have these disorders according to Dr. So and So document.

That is where I was diagnosed! :)
So I can tell you exactly what your report will consist of (presuming you had the same doctor).

The report is basically a full write-up of the assessment, including a list of early childhood indicators, psychiatric history, education history etc, then detailed explanations of how you meet criteria under headings such as "Communication", "Repetitive Behaviours" etc. It ends with a list of recommendations, such as books you might want to read, and modifications that people should make for you in light of the diagnosis.

Hope that helps.
