I have a mental health caseworker that visits my home. I realized I hand flapped in front of her and she was talking about handling my anger at work i described the shutdowns I had at work.

Should someone like this recognize red flags (she randomly mentioned to me on a previous occasion if I noticed the big changes DSM 5 brought to autism) or am i going to assume they are going to continue focusing on my mood disorder, they are already working on my severe meltdowns but do not attribute them to autism.

Most of these case workers are pretty clueless about autism. If she thinks your meltdowns are not the result of your autism she is clueless to the extreme. That she heard about the DSM 5 does not prove expertise. Maybe she heard about it from a coworker or read about it somewhere.

Request another caseworker.
