I apologize if this is in the wrong place...

If you were a former applicant of vocational rehab in any state but are no longer going through the program, would it be possible for them to send you or a relative copies of any psychological assessments given to them? The thing is that I went through vocational rehab up in Montana and I forgot to ask for a copy of my psychological evaluation and I now live in another state and I need that report to prove that I have a learning disorder among other things.

If I were you I would explain them your situation and ask for one.

I don't have any experience with your exact situation, but my experience with psychiatric reports is, that sometimes you get them, sometimes you don't and if I don't get them, they are usually willing to send them at least to my GP.

The thing is when I called the psychologist's office and left a message asking about my evaluation, I gave them my current home number and asked for them to call me back, no one bothered. I'm hoping that my mother can obtain a copy and send it to me so that I can have some proof of my NVLD for student disability services or medical purposes.

Per HIPPA they have to provide you with them. I think someone got busy and forgot to call you back.

And. If it was your mother who called, that's probably your problem right there...you are 23 so it is illegal to provide her with your medical information unless you fill out a release form authorizing them to do so.

My mother didn't contact anyone regarding it.
