Question about the cover: Did you design your book covers yourself, or did you pay someone to design it? Even if the book is entirely virtual, you still needed that professional-looking thumbnail cover. Who designed it?

Also, what software do you use to write/edit the book, and what software do you use to eventually upload it?

Actually, I lucked out having a fellow writer as a friend who is also something of a graphics designer (I think that's the right term). He made up a cover for me in return for a couple beers.

No, another writer I know, who also does his own covers, did my first two. At the time, he wanted to help out another writer. My friend simply is easier to get hold of, so I've been accepting his help these days.
